Stormwater Program for Municipal Separate Storm Sewer Systems (MS4): Annual Reports

MS4 Annual Reports are to be completed and submitted to the Minnesota Pollution Control Agency (MPCA) by June 30 of each year, detailing permit activities for the previous calendar year. The 2011 Annual Report is due June 30, 2012, and covers permit activities from January 1, 2011, through December 31, 2011.

2011 Report Form

The MPCA has published the 2011 MS4 annual report form; see Cover Letter for instructions on completing the form. As noted above, the Annual Report is due June 30, 2012.
