Forms

PY5 reporting forms

Program Year 5 (July 1, 2011, through June 30, 2012) reporting is required for all registered stakeholders.

Registered admin contacts were notified by e-mail about the PY5 reporting forms and what's required to register for PY6.

Manufacturer reporting forms and resources for online payment are administered by the Department of Revenue web site.

PY6 Registration

Registration must be completed each program year.

Manufacturer registrations were due Sept. 1, 2012; forms were e-mailed to registered administrative contacts. The required registration form details company contacts and information on brand/device types. Registration for program year 4 requires payment of the base registration fee as calculated using the reporting form EW-1 [see Manufacturer Reporting Resources (Dept of Revenue)].

Collectors and recyclers registered in PY5 were e-mailed an online confirmation message to review the old information and make updates as necessary. If you did not receive this message, contact MPCA for assistance: electronics.pca@state.mn.us or 651-757-2211, 800-657-3864 toll free.

New registrations should use these forms to submit registration information electronically via e-mail; print a copy for your records.
